摘    要:目的:加强医院化验标本运送的管理,规范化验标本运送的流程,避免化验标本运送错漏或不及时。方法:成立失效模式与效应分析(FMEA)小组,对化验标本运送进行风险评估。通过执行分析,确定4项影响标本准确、及时运送的潜在失效因素为高风险因子,制定、实施改进措施。结果:标本运送缺陷发生率由6.50%下降至1.27%,差异有统计学意义(P<0.01)。4项潜在失效因素的风险优先指数由1550下降至250。结论:将FMEA模式应用于医院化验标本运送管理中,能前瞻性地发现流程中潜在的漏洞,降低标本运送错漏发生率,提高标本送检的准确性和及时性。

Abstract:Objective: To strengthen the management of laboratory specimen delivery in hospital, standardize transportation processes, and avoid shipping errors or not timely. Methods: Establish failure mode and effects analysis (FMEA) team; assess the risk of specimen transportation; performing analysis; determine the four potential failure factors influencing accurate and timely delivery of the specimen; calculate risk priority number; and develop improvement measures. Results: The incidence rate of transporting specimens errors decreased from 6.50% to 1.27% (P0.01). Priority Index (RPN value) of four potential failure factors decreased from 1550 to 250. Conclusion: FMEA model used in the management of hospital laboratory specimen transportation, can proactively identify potential vulnerabilities, reduce the incidence of errors and omissions, and improve the accuracy and timeliness.
