摘    要:目的 探讨3.0T磁共振灌注加权成像(PWI)在脑胶质瘤的分级及与单发转移瘤鉴别诊断巾的价值.方法 对经病理证实的低级别胶质瘤17例、高级别胶质瘤15例、单发转移瘤10例行常规磁共振成像及灌注加权成像,观察肿瘤的时间-信号强度曲线及rCBV值.结果 高、低级别胶质瘤的时间-信号强度曲线不同.在肿瘤实质区域,低级别胶质瘤rCBV值(1.974±0.539)低于高级别胶质瘤(5.785+1.223)和单发转移瘤(5.324±1.008)(P<0.05);在瘤周水肿区域,单发转移瘤rCBV值(0.387±0.129)低于高级别胶质瘤(1.481±0.204)(P<0.05).结论 PWI可用于高、低级别胶质瘤的分级及与单发转移瘤的鉴别诊断.

The value of 3.0T MR perfusion-weighed imaging in diagnosis of brain gliomas
Abstract:Objective To evaluate the value of MR perfusion-weighted imaging(PWI) of 3.0T in grading brain gliomas and differential diagnosis of gliomas and solitary metastatic tumors.Methods Conventional MR imaging and PWI were performed in 17 cases of low-grade gliomas, 15 cases of high-grade gliomas,and 10 cases of solitary metastatic tumors.All cases were pathologically proved after operation.The time-signal curves and the rCBV value were observed.Results High-grade gliomas showed different time-signal curves from low-grade gliomas.In the tumor parenchyma area, the rCBV value of low-grade gliomas was(1.974±0.539), which was lower than(5.785±1.223) of high-grade gliomas and(5.324± 1.008)of metastatic tumors(P<0.05).In the peritumoral edema area.the rCBV value of metastatic tumors was(0.387±0.129) ,which was lower than(1.481±0.204) of high-grade gliomas(P<0.05).Conclusion PWI can be used in grading brain gliomas and differentiating gliomas from solitary metastatic tumors.
