超声引导下自动活检在胸部疾病中的诊断价值

目的探讨其他检查方法对胸部疾病不能做出定性诊断而行超声引导下自动活检穿刺的诊断价值。方法超声对胸部外周性疾病检查并确认穿刺路径后，直接在超声引导下用美国产Bard自动活检枪及18G、19G内槽式活检针对病灶区域进行穿刺活检，并对所取组织做病理学检查。结果本组107例全部穿刺成功，病理证实肺癌57例(53％)，炎症21例(19．6％)，结核4例(3．7％)，正常肺组织7例(6．5％)，纵隔胸腺瘤6例(5．6％)，纵隔淋巴瘤2例(1．86％)，纵隔原始神经外胚层瘤1例(0．93％)等等。出现轻度咳血及痰中带血13例(12．19％)，无气胸及大咳血严重并发症。结论超声引导下自动活检对胸部疾病可做定性诊断，为临床治疗提供可靠的依据，具有较高的临床应用价值，其方便、实用、并发症少，可做为胸部外周疾病的首选检查方法。

Value of Ultrasound Guided Automatic Biopsy in Thoracic Diseases

Objective To assess the diagnostic value of ultrasound guided automatic biopsy (USGAB) in thoracic diseases which could not be diagnosed accurately with other methods. Methods We tested the peripheral thoracic diseases confirmed the track under ultrasound guided Bard automatic biopsy gun (made in America)and 18G,19G tru-cut biopsy needles were used and the core tissues were proved by pathology. Results One hundred and seven cases were all successful. The pathology revealed that 57 cases(53%)had pulmonary careinoma;21 cases (19. 6%) Inflammation;4 cases(3. 7%) tuberculosis; 7 cases (6. 5%) ,natural pulmonary tissues ; 6 cases (5. 6%) , mediastinal thymic tumors;2 cases (1. 86%) ,mediastinal lymphomas;1 case (0. 93%) ,primitive neuroectodermal tumor etc. All of the cases had no acute complications, but sputum, only 13 cases (12.19%) appeared mild hemoptysis or bloodstained. Conclusions The peripheral thoracic diseases can be diagnosed correetly with USGAB,which can give actual evidence for clinical and can be the first test method of peripheral thoracic diseases.
