乙肝表面抗原阳性孕妇胎盘组织中bcl-2、bax和HBcAg的表达

摘    要:目的 通过检测乙肝表面抗原(HBsAg)阳性孕妇胎盘组织细胞中bcl-2、bax基因及HBcAg的表达,探讨bcl-2、bax与HBcAg之间的关系。方法 46例HBsAg阳性孕妇足月产胎盘(排除其他疾病)为HBV组,25例HBsAg阴性(HBV-DNA阴性)足月产妇胎盘做为对照组。HBV组中根据HBcAg结果分为阳性组和阴性组。采用免疫组化法测定两组孕妇胎盘组织细胞中bcl-2、bax、HBcAg的表达。结果 (1)HBV组和对照组比较bcl02基因表达无统计学意义(P〉0.05),bax基斟表达阳性率明显高于对照组,两者比较有统计学意义(P〈0.05);(2)胎盘组织细胞中HBcAg的表达:HBcAg阳性信号表达于胎盘各层细胞中,滋养层细胞感染率为41.3%,绒毛问质细胞感染率为15.2%,绒毛毛细血管内皮细胞感染率为6.5%;(3)HBcAg阳性组与HBcAg阴性组中bcl-2基因表达比较无统计学意义(P〉0.05),bax基因表达在HBcAg阳性组明显高于HBcAg阴性组,有统计学意义(P〈0.05)。结论 HBV感染孕妇胎盘组织中bax表达明显升高,提示bax可能参与了胎盘组织细胞凋亡的过程;HBsAg阳性孕妇胎盘中HBcAg可能是逐层感染胎盘各型细胞,且胎盘组织中HBcAg阳性者bax表达与HBcAg关系密切。

Abstract:Objective To investigate the expressions of bcl-2, bax and HBcAg in placenta tissues of HBsAg positive pregnant women and its relationship with HBV intrauterine transmission. Methods Bcl-2, bax or HBcAg expression in placenta was detected by immunohistochemistry in 46 placental samples with positive HBsAg and 25 normal samples. Results Significant differences of bax expression were found between HBV and control groups(P < 0.05), but bcl-2 expression had no differences between the two groups(P > 0.05). The same results were found in HBcAg positive and negative groups. HBcAg was mainly located in the cytoplasm of infected placenta cells. HBV infection rates in trophoblast cells, villous mesenchymal cells, and villous capillary endothelial cells were 41.3%, 15.2% and 6.5% respectively. Conclusion bax was highly expressed in HBV infected placental cells, and it might participate in apoptosis process of placental cells. HBcAg was found in different placental cells of HBsAg positive pregnant women, and the high expression of bax might be closely related to HBcAg infection.
