斜仰截石位经皮肾镜取石术在肾结石手术中的应用效果研究

目的探讨斜仰截石位经皮肾镜取石术(PCNL)在肾结石手术中的应用效果。方法选取我院行斜仰截石位PCNL的219例肾结石患者作为研究组,同时选取100例行传统开放取石术治疗的肾结石患者作为对照组,比较两组患者的手术指标、临床指标、结石清除率。结果研究组的手术时间、住院时间显著短于对照组,术中出血量显著少于对照组(P均<0.05)。术前,两组的肾积水、肾盂集合系统分离指数、血尿素氮、血肌酐水平比较差异无统计学意义(P>0.05);术后3个月,研究组的肾积水、肾盂集合系统分离指数、血尿素氮、血肌酐水平显著低于对照组(P <0.05)。两组患者出院时的结石完全清除率比较无统计学差异(P>0.05)。结论斜仰截石位PCNL应用于肾结石手术中,可缩短患者的手术时间和住院时间,减轻其创伤。

Study on Application Effect of Percutaneous Nephrolithotomy with Oblique Lithotomy Position in Kidney Stone Surgery

Objective To explore the application effect of percutaneous nephrolithotomy(PCNL) with oblique lithotomy position in kidney stone surgery. Methods 219 kidney stone patients undergoing PCNL with oblique lithotomy position in our hospital were selected as study group. At the same time, 100 patients with kidney stone undergoing traditional open lithotomy were selected as control group. The surgical indicators, clinical indicators and stone removal rate of the two groups were compared. Results The surgical time and hospitalization time of the study group was significantly shorter than that of the control group, and the intraoperative blood loss was significantly less than that of the control group(all P <0.05). Before surgery, no statistical difference was found in the hydronephrosis,separation index of renal pelvis collection system, blood urea nitrogen and blood creatinine levels between the two groups(P >0.05). 3 months after surgery, the hydronephrosis, separation index of renal pelvis collection system, blood urea nitrogen and blood creatinine levels of the study group were significantly lower than those of the control group(P <0.05). No statistical difference was found in the complete stone removal rate at discharge between the two groups(P >0.05). Conclusions PCNL with oblique lithotomy position in kidney stone surgery can shorten the operation time and hospitalization time, and alleviate the trauma of patients.
