丹参川芎嗪注射液治疗慢性肺源性心脏病急性发作期合并左心衰竭的临床效果探讨

目的探讨丹参川芎嗪注射液治疗慢性肺源性心脏病急性发作期(AECPHD)合并左心衰竭的临床效果。方法将150例AECPHD合并左心衰竭患者随机分为干预组和对照组,各75例。对照组给予解痉、消炎、平喘、利尿、吸氧、强心及纠正电解质紊乱等常规治疗,干预组在对照组基础上采用丹参川芎嗪注射液治疗,两组均治疗14 d。比较两组的治疗效果、治疗前后血气分析相关指标(血氧分压、二氧化碳分压)及心功能相关指标(心率、左心室射血分数、左心室舒张末期内径、左心室收缩末期内径)。结果干预组的治疗总有效率高于对照组(P <0.05)。治疗后,干预组的血气分析相关指标及心功能相关指标均明显优于对照组(P <0.05)。结论在常规治疗基础上采用丹参川芎嗪注射液治疗AECPHD合并左心衰竭患者,可显著改善其血气情况及心功能,临床疗效显著。

D iscussion on Clinical Effect of Salviae Miltiorrhizae and Ligustrazine Hydrochloride Injection in the Treatment of Acute Exacerbation of Chronic Pulmonary Heart Disease Complicated with Left Heart Failure

Objective To explore the clinical effect of salviae miltiorrhizae and ligustrazine hydrochloride injection in the treatment of acute exacerbation of chronic pulmonary heart disease(AECPHD) complicated with left heart failure. Methods 150 cases of patients with AECPHD complicated with left heart failure were randomly divided into intervention group and control group, with 75 cases in each group.The control group was given routine treatment such as spasmolysis, anti-inflammation, antiasthmatic, diuretic, oxygen inhalation, cardiotonic and correction of electrolyte disorders. The intervention group was treated with salviae miltiorrhizae and ligustrazine hydrochloride injection on the basis of the control group. Both groups were treated for 14 days. The treatment effect and the related indicators of blood gas analysis(blood oxygen partial pressure, carbon dioxide partial pressure) and heart function(heart rate, left ventricular ejection fraction, left ventricular end-diastolic diameter, and left ventricular end-systolic diameter) before and after treatment were compared between two groups.Results The total effective rate of treatment in the intervention group was higher than that in the control group(P <0.05). After treatment, the related indicators of blood gas analysis and heart function in the intervention group were significantly better than those in the control group(P <0.05). Conclusions Salviae miltiorrhizae and ligustrazine hydrochloride injection on the basis of routine treatment in patients with AECPHD complicated with left heart failure can significantly improve the blood gas and heart function, with significant clinical effect.
