牙周非手术治疗对2型糖尿病伴牙周病中老年患者糖化血红蛋白的影响

摘    要:目的探讨牙周非手术治疗对2型糖尿病伴牙周病中老年患者糖化血红蛋白的影响。方法选择2018年3月至2019年4月期间在我院行牙周非手术治疗的2型糖尿病伴牙周病中老年患者50例作为观察组,另选择同期行牙周非手术治疗的非2型糖尿病伴牙周病中老年患者50例作为对照组。观察两组患者治疗前后的牙周指标、糖化血红蛋白和炎性因子水平。结果治疗后3个月,两组的菌斑指数、牙周袋探诊深度和龈沟出血指数低于治疗前,但观察组的菌斑指数、牙周袋探诊深度和龈沟出血指数高于对照组(P均<0.05)。治疗后3个月,观察组的HbA1c、 TNF-α、 CRP水平低于治疗前,但仍高于对照组(P均<0.05)。结论牙周非手术治疗可有效改善2型糖尿病伴牙周病中老年患者的病情,降低糖化血红蛋白水平,但疗效不如非2型糖尿病患者,应做好相关监测与治疗。

Influence of Non-Surgical Periodontal Treatment on Glycosylated Hemoglobin in Middle and Old Aged Patients with Type 2Diabetes Mellitus Complicated with Periodontal Disease

Abstract:Objective To explore the influence of non-surgical periodontal treatment on glycosylated hemoglobin in middle and old aged patients with type 2 diabetes mellitus complicated with periodontal disease. Methods 50 middle and old aged patients with type 2 diabetes mellitus complicated with periodontal disease who received periodontal non-surgical treatment in our hospital from March 2018 to April 2019 were selected as observation group, and another 50 middle and old aged patients with non-type 2 diabetes mellitus complicated with periodontal disease who received non-surgical periodontal treatment during the same period were selected as control group. The periodontal index, and the levels of glycosylated hemoglobin and inflammatory factors before and after treatment of the two groups were observed. Results 3 months after treatment, the plaque index, periodontal pocket probing depth and sulcus bleeding index of the two groups were lower than those before treatment, but the plaque index, periodontal pocket probing depth and sulcus bleeding index of the observation group were higher than those of the control group(P <0.05). 3 months after treatment, the HbA1 c, TNF-α and CRP levels of the observation group were lower than those before treatment, but higher than those of the control group(P <0.05). Conclusions Non-surgical periodontal treatment can effectively improve the disease condition of middle and old aged patients with type 2 diabetes mellitus complicated with periodontal disease, and reduce the glycosylated hemoglobin level, but the curative effect is not as good as that of patients with non-type 2 diabetes mellitus. Therefore, the related monitoring and treatment is necessary.
