P53蛋白在喉鳞状细胞癌和不典型增生中的表达及临床意义

目的 :探讨 P53蛋白表达在癌前病变早期定性诊断的价值和在喉癌发生中的作用机理。方法 :应用 P53单克隆抗体 ( DO- 1 )对 1 9例喉癌鳞状细胞癌、34例喉粘膜不典型增生上皮和 1 7例声带息肉进行免疫组织化学染色。结果 :显示 P53蛋白阳性物质位于癌细胞核内和转化的细胞核内。在 1 9例喉癌中 P53蛋白表达阳性者 1 4例 ,表达率为 73.68%;在 34例不典型增生上皮中有2 0例 P53蛋白表达阳性 ,表达率为 58.82 %,在 1 7例声带息肉中未见有 P53蛋白的表达。结论 :P53蛋白免疫组化可以做为临床上癌前病变早期定性诊断和喉癌早期诊断的指标之一

P53 protein expression in laryngeal squamous cell carcinoma and dysplasia

Objective:The purpose of this article was to discuss the value and the machenism of P53 protein expression in quantitative diagnosis of premalignant lesions and in the development of cancer.Methods:We analyzed the P53 protein immunohistochemically in laryngeal squamous cell carcinoma (19cases),dysplasias (34 cases) and vocal cords polyps (17 cases).Results:Fourteen of the 19 cases with carcinoma showed P53protein expression positive (73.68%).Twenty of the 34 cases with dysplasias showed P53 protein expression positive (58.82%) and all the polyps were negative.The P53 antigenicity was generally located in the nucli of malignant cells and the transformed cells of the basal part of epithelia.There were statistically differences in the three groups by X 2 test.Conclusion:These findings indicate that P53 protein expression is possible in premalignant laryngeal lesions and suggest that P53 expression could be an early event in the development of squamous cell carcinoma of larynx at least in some cases.
