高血压脑出血的早期微创手术治疗优势

目的 比较微创手术与传统手术对早期高血压脑出血患者的治疗效果,探讨早期微创手术在高血压脑出血治疗中的优势.方法 收集我院2009-2011年高血压脑出血病发8～16h内进行手术治疗的患者116例,其中微创手术57例,传统开颅减压术59例,对两组患者的近期疗效(1个月内)及远期疗效(3个月内)进行评定,并利用统计学软件对所有数据进行统计学分析.结果 早期疗效评价结果显示微创组的1～4级指标比率为31.6％,68.4％的患者属于第5级(恢复良好)；传统组1～4级指标比率为49.2％,50.8％的患者属于第5级,两组数据比较差异具有统计学意义(P＜0.05).远期疗效评价结果显示微创组的优良率(79.2％)高于对照组(66％),其差异亦具有统计学意义(P＜0.05).结论 早期微创手术在高血压脑出血的治疗中可明显提高治疗效果,具有明显的优势,值得推广.

Advantages of early minimally invasive surgery in the treatment of hypertensive intracerebral hemorrhage.

Objective To discuss the advantages of early minimally invasive surgery in the treatment of hypertensive intracerebral hemorrhage by comparison with the traditional operation. Methods The clinical data of 116 patients with hypertensive intracerebral hemorrhage undergoing surgery within 8~16 hours were collected, including 57 patients treated by minimally invasive surgery (group A) and 59 patients treated with traditional operation (group B). The curative effect within one month and three months were evaluated. All the data were statistically analyzed by SPSS13.0. Results In group A, the curative effect within one month were evaluated as grade 1~4 in 31.6% patients and as grade 5 in 68.4% patients. In group B, the curative effect within one month were evaluated as grade 1~4 in 49.2% patients and as grade 5 in 50.8% patients. The short-term effect in group A was significantly better than that in group B. The excellent and good rate of the curative rate within three months was 79.2% in group A and 66.0% in group B, showing statistically significant difference. Conclusion For treating hypertensive intracerebral hemorrhage, early minimally invasive surgery can promote the curative effect and have many advantages, which is worthy of application in clinic.
