右心室间隔部起搏对心功能影响的临床研究

目的 比较右室间隔(RVS)和右室心尖(RVA)起搏对心功能的影响.方法 选择天长市人民医2014年7月至2015年11月收治的需要置入永久起搏器的患者54例,随机分A组(RVS起搏,26例)和B组(RVA起搏,28例),比较两组患者的起博参数、术后1个月、3月和6月心功能指标和NT-proBNP差异.结果 两组患者起博参数差异无统计学意义(p＞0.05).A组术后3个月和6个月的QRS时限和左室舒张末期内径低于B组,而左室射血分数高于B组,差异有统计学意义(P＜0.05).两组术后1周、1个月、3个月和6个月的NT-proBNP差异无统计学意义(P＞0.05).结论 与RVA起博相比,RVS起博能够改善患者的左室功能和重构.

The clinical study of effect of right ventricular septal pacing on cardiac function in patients implanted with pacemakers

Objective T0 observe the effect of right ventricular septal (RVS) pacing on cardiac function compared to right ventricular apical pacing in patients implanted with pacemakers.Methods Fifty-four patients were randomly divided into two groups:group A received RVS pacing (n =26) and group B received right ventricular apical (RVA) pacing (n =28).The pacing parameters,cardiac function indexes,and NT-proBNP between the two groups were compared.Results There was no significant difference in pacing parameters between group A and group B (P ＞ 0.05).The mean QRS duration and LVEDD of group A were lower,and LVEF was higher than those of group B 3-month and 6-month after procedure (P ＜ 0.05).The piasma NT-proBNP levels between the two groups were comparable 1-week,1-month,3-month and 6-month after pacemaker implantation (P ＞ 0.05).Conclusion Compared with RVA pacing,RVS pacing improves the left ventricular systolic function and remodeling.
