静注异搏定后室上性心动过速终止前RR周期长短交替的机理与临床意义

摘    要:目的 :观察到 3 0例房室外反复性心动过速、12例房室内折返性心动过速静注异搏定后 ,心动过速终止前RR周期长短交替发生率分别为 3 7%、3 3 % ,长短差值 5 0ms仅见房室反复性心动过速。对比房室结电生理特点 ,认为室上速RR周期长短交替是冲动分别落在房室结相对不应期的不同阶段或经房室结不同经路下传达室的结果

Abstract:Objective:It was reported that the alternation of RR cycle length,among 11 of 30 patients(37%) with AV reciprating tachycardia(AVRT) and 4 of 12 with AV node reentry tachycardia(AVNRT),before reversion to sinus rhythm occured after the administration of verapamil.The difference was 50ms between the long and the short cycle length occdurred only among the patients with AVRT.In consideration of the electrophysiologic characteristics of the AV node,it is concluded that the cycle length alternation during PSVT may be attributed to the pulse conduction at different refractoriness and/or via different rathways of the AV node.
