| 摘 要： | 目的：探讨伴退变性脊髓压迫的神经根型颈椎病患者的治疗方法及转归。方法：29例伴颈椎退变性脊髓压迫的颈神经根病患者纳入研究,全部患者入院后继续进行2周系统、综合的非手术治疗,如果神经功能继续损害或疼痛剧烈不能缓解,进行手术治疗,分别于治疗前、治疗后2周、随访期间通过VAS及JOA评分评价临床疗效。结果：29例患者平均随访1.16年,其中18例获得不同程度的改善,保守治疗改善不明显者11例,其中6例行手术治疗,余5例患者拒绝手术治疗;随访期间,2例保守治疗无效患者无改善,但未明确出现脊髓型受压的症状体征,5例症状逐渐缓解,痹痛减轻。治疗前后及随访期间该组患者疼痛及JOA评分改善情况比较差异有统计学意义。结论：伴颈椎退变性脊髓压迫的神经根型颈椎病患者经过系统、综合的非手术治疗后存在不同转归,部分可以＂治愈＂,部分仍需手术治疗,不能简单的将＂颈神经根病＂作为无脊髓症状颈椎退变性脊髓压迫向脊髓型颈椎病转化的信号。
