剖宫产率与社会因素的临床探讨

目的：探讨近年来剖宫产率上升的原因,剖宫产指征的变化,尤其社会因素的增长,寻找降低剖宫产率的有效措施。方法：选择2003年至2007年分娩的2045例产妇（阴道分娩1084例,剖宫产961例）,其中选择2006年1月至2007年12月之间分娩的997例产妇（阴道分娩489例,剖宫产508例,其中社会因素剖宫产占258例）,回顾性分析剖宫产率的变化及相关因素。结果：5年来剖宫产率呈明显上升趋势,在剖宫产指征中社会因素占首位（50.8%）,其次为胎位异常、头盆不称、胎儿窘迫等。结论：正确掌握剖宫产指征,做好产前宣传教育,有效降低社会因素为指征的剖宫产,而降低剖宫产率,减少产后并发症的发生。

Clinic exploration of caesarean operation rate and social factors

Objective To explore the increasing cause of caesarean operation in the recent years,and changes of the indication of caesarean operation,especially increasing of the social factor,and look for the effective measures to reduce caesarean operation rate.Method Total 2 045 patients who delivered in Antu hospital from 2003 to 2007(vagina delivery 1 084 cases,caesarean operation 961 cases) were selected.For 997 cases which delivered from January 2006 to December 2007(vagina delivery 489 cases,caesarean operation 508 cases,social factor-caesarean operation 258 cases) were selected.Retrospective analysis was made on changes of caesarean operation rate and relative factors.Results The caesarean operation rate showed an obvious increasing tendency in recent 5 years.The social factor-caesarean operation became the first cause,and counted for 50.8%.The others were fetal position abnormal,cephalopelvic disproportion,fetal distress and so on.Conclusion We must grasp the indication of caesarean operation correctly,provide rich antepartum education,and decreasing the social factor-caesarean operation effectively,to reduce the caesaren operation rate and lower the incidence of postpartum complication.
