外侧裂区胶质瘤临床特点及手术方法

目的：探讨外侧裂区胶质瘤的临床特点、影像学特征以及手术方法。方法：对42例手术证实为外侧裂区胶质瘤的患者资料进行回顾性分析。结果：全切30例，次全切12例，无手术死亡病例。结论：外侧裂区胶质瘤青壮年多见，病程相对较长，临床症状以头痛、癫痫发作为主，有定位体征倒不足1／3。翼点入路暴露较好，术中对大脑中动脉的分支和伴行的大脑中静脉的主干以及重要功能区的保护是手术成功的关键。

The clinical characteristics and the operation techniques of gliomas located in lateral fissure area

Objective To investigate the clinical charactistics and operation techniques of the gliomas located in lateral fissure area.Methods The data of 42 patients with gliomas located in lateral fissure proved by operation were analyzed retrospectively.Results Of 42patients with the gliomas located in lateral fissure area,30 cases received total resection,12 cases received subtotal resection.No patients died from the operation.Conclusion The gliomas located in lateral fissure area occures chiefly in younth and middle-aged patients.The initial sign was headache in 23 cases,the initial sign was epilepsy in 16 cases.As the initial sign,focal motor signs or abnormalities of sensation were uncommon.The pterional approach is well suited to resection of the gliomas located in lateral fissure.For the operations succeed,it is vitally important that the main branches of middle cerebral artery and the important brain functional areas were preserved.
