经皮椎体成形术及经皮椎体后凸成形术治疗骨质疏松性椎体压缩性骨折的临床疗效对比

目的：探讨经皮椎体成形术（ PVP ）及经皮椎体后凸成形术（ PKP ）治疗骨质疏松性椎体压缩性骨折（ OVCF）的临床疗效。方法选取2011年4月～2013年2月在我院治疗的骨质疏松性椎体压缩性骨折（ OVCF）患者68例，将患者随机分为PVP组和PKP组，观察两组手术前后视觉模拟评分（ VAS）、椎体高度压缩率、骨水泥渗漏、住院费用等指标差异。结果 PKP组手术时间和骨水泥注入量分别为（39．72±6．82）min／椎和（6．11±1．73）mL／椎，均明显高于PVP组（P＜0．05）； PVP组和PKP组术后VAS评分均较术前明显改善，但PVP组和PKP组各时点VAS评分差异无统计学意义（ P＞0．05）； PVP组和PKP组术后椎体高度压缩率均较术前明显改善，但PKP组术后各时点椎体高度压缩率均低于PVP组（ P＜0．05）； PKP组骨水泥渗漏率为25．53％，明显低于PVP组的46．51％，差异有统计学意义（ P＜0．05）； PVP组患者平均住院费用为（1．26±0．11）万元，明显低于PKP组患者的（4．97±0．23）万元，差异有统计学意义（P＜0．05）。结论 PVP和PKP治疗OVCF，均可以有效缓解疼痛，PKP有利于椎体高度的恢复，较少发生骨水泥渗漏情况，但费用较高。

Comparison of the clinical efficacy of percutaneous vertebroplasty and percutaneous kyphoplasty in the treatment of osteoporotic vertebral compression fracture

Objective To investigate and compare the curative effect of percutaneous vertebroplasty( PVP) and percutaneous kyphoplasty(PKP)in the treatment of osteoporotic vertebral compression fracture(OVCF).Meth-ods A total of 68 patients diagnosed with OVCF and hospitalized from Apr.2011 to Feb.2013 in our hospital were randomly divided into PVP group and PKP group.Visual analogue scale(VAS),vertebral compression rate and oth-er indicators were observed and compared between the two groups.Results The operation time and bone cement injection amount were (39.72 ±6.82) min/vertebra and (6.11 ±1.73) mL/vertebra in PKP group,respectively, which were significantly higher than those of PVP group(P<0.05); The VAS scores were significantly improved compared with preoperative ones in both PVP group and PKP group,but the VAS scores at each time point had no significant difference between the two groups( P<0.05); The vertebral compression rate of the two groups was sig-nificantly improved compared with preoperative,but the vertebral compression rate of the PKP group was lower than that of PVP group at each time point( P<0.05); The cement leakage rate of PKP group (25.53%) was signifi-cantly lower than that of the PVP group(46.51%)and the difference was statistically significant(P<0.05).The average hospital cost of PVP group was (10.26 ±1.1) thousand yuan and significantly lower than that of PKP group (4.97 ±0.23) thousand yuan,P<0.05].Conclusion Both PVP and PKP can effectively relieve pain of the OVCF patients.PKP is more conducive to the restoration of vertebral height and leads to less bone cement leakage but demands higher hospital cost.
