摘    要:目的: 探讨MSCT 3D成像在肾上腺病变腹腔镜手术中的应用价值.材料和方法: 对54例肾上腺病变和10例正常肾上腺进行薄层扫描.采用Marconi-MX8000型CT机的工作站及其工作软件对图像进行3D重建,包括多平面重建(MPR)、容积再现(VR)及透明重建.对照手术结果分析3D重建图像对肾上腺病变腹腔镜术中的应用价值.结果: 54例肾上腺病变中,皮质腺瘤35例,嗜铬细胞瘤11例,髓性脂肪瘤4例,囊肿4例.MPR、VR及透明重建的图像均清晰地显示肾上腺病变的立体结构及其毗邻器官、血管的关系.在3D图像指导下全部病例均成功地实施了后腹腔镜切除术.结论: 3D成像可立体直观地显示肾上腺病变的形态轮廓、大小及其邻近器官、血管的关系,为腹腔镜手术提供极有效的帮助.

Abstract:Pupose: To study the applicable value of MSCT 3D reconstruction for adrenal lesion operation by laparoscopy. Materials and Methods: 10 normal volunteers and 54 patients with adrenal lesions were examined by MSCT and three-dimensional(3D) reconstruction including MPR,VR and transparency,at Marconi-MX8000 system was done.The applicable value of 3D reconstruction was studied compairing with laparoscopy operative results. Results: Among 54 lesions there were 35 adrenal adenomases, 11 pheochromocytomases, 4 adrenal myelolipomas, 4 adrenal cysts. The images of MPR,VR and transparency could clearly show the normal adrenal gland and adrenal lesions?including their outline ,shape,size and relationship to near organs and vessels. Under the guide of the 3D images all adreal lesion were successfully operated by laparoscopy.Conclusion: MSCT 3D reconstruction can clearly,stereoscopically display the shape,outline,size and neigbourhood of adrenal gland and lesions,and effectively guide to operate by laparoscopy.
