米索前列醇与COOK球囊对低宫颈评分孕妇促宫颈成熟疗效分析

摘    要:目的探讨米索前列醇与COOK宫颈球囊对低宫颈评分孕妇促宫颈成熟效果及引产结局的影响。方法选取深圳市宝安区松岗人民医院2015年5月至2017年1月有引产指征的头位初产妇400例,根据采用的促宫颈成熟方法分为米索前列醇组与COOK球囊组,各200例。观察两组治疗前后Bishop评分、阴道分娩率、引产成功率、产褥感染率、母婴妊娠结局、不良事件发生率等。结果促宫颈成熟前米索前列醇组与COOK球囊组Bishop评分分别为(1.99±0.29)分和(1.95±0.34)分,差异无统计学意义(P﹥0.05)。两组促宫颈成熟治疗后Bishop评分分别为(5.98±1.26)分和(4.70±1.40)分;阴道分娩率、引产成功率及总阴道分娩率分别为9.00%、66.48%、69.50%和2.00%、47.45%、48.50%,米索前列醇组均高于COOK球囊组,差异有统计学意义(P<0.05)。两组引产失败后分娩结局比较差异无统计学意义(P>0.05);所有孕产妇均未发生产褥感染及不良事件,母婴均未出现不良妊娠结局。结论米索前列醇对低宫颈评分孕妇促宫颈成熟疗效优于COOK球囊。

Analysis of the effect of misoprostol and COOK cervical balloon on the treatment of cervical ripening in low Bishop score pregnant women

Abstract:Objective To investigate the effect of misoprostol and COOK cervical balloon on cervical ripening and induction of labor outcome in pregnant women with low cervical score.Methods 400 cases of primiparas with indications for induction of labor from May 2015 to January 2017 in Songgang People’s Hospital of Bao’an District were selected.According to the method of promoting cervical ripening, the patients were divided into misoprostol group and COOK balloon group.200 cases in each group.The Bishop score, vaginal delivery rate, induction rate, puerperal infection rate, maternal and fetal pregnancy outcome, and incidence of adverse events were observed before and after treatment.Results The Bishop scores of the misoprostol group and the COOK balloon group before the cervical ripening were(1.99±0.29) and(1.95±0.34), respectively, and the difference was not statistically significant(P>0.05).And Bishop scores of the misoprostol group and the COOK balloon group were(5.98 ±1.26) and(4.70± 1.40), respectively after cervical ripening, the vaginal delivery rate, induction rate and total vaginal delivery rate were 9.00 %, 66.48 %, 69.50 % and 2.00 %, 47.45 %, 48.50 %, respectively.The misoprostol group were higher than the COOK balloon group.The differences were statistically significant(P<0.05).There was no significant difference in the outcome of delivery between the two groups after induction of labor failure(P>0.05).All pregnant women had no sputum infection and adverse events, and no adverse pregnancy outcomes were found in both mother and child.Conclusion Misoprostol is superior to COOK balloon in promoting cervical ripening in pregnant women with low cervical score.
