复发性流产患者社会心理相关因素的Logistics回归分析

目的探究复发性流产(recurrent spontaneous abortion,RSA)患者社会心理相关危险因素,为其临床干预及治疗提供科学依据。方法选取2018年2月至2019年2月在兰州大学第二医院就诊的659例育龄期女性进行病例对照研究,通过问卷调查收集研究对象的社会心理相关资料,对2018年2月至2018年10月收治的216例RSA患者(连续发生2次及以上自然流产)和224例非RSA患者(既往无自然流产,且至少有1次正常妊娠并成功分娩)进行数据分析,建立Logistic回归模型(logistic regression model,LRM),采用受试者工作特征曲线(receiver operating characteristic curve,ROC)评估模型的预测价值,并在2018年11月至2019年2月收治的患者中进行验证。结果LRM包括年龄、受教育程度、职业、化妆、接触厨房油烟、夜间睡眠时间、夫妻性生活频率、人际关系及有无抑郁症状,总准确率为65.2%,敏感性为69.4%,特异性为61.2%。随后,对LRM进行了验证,总体准确度为74.4%。结论LRM是预测RSA患者社会心理相关危险因素的一个有价值的模型。

Logistic regression analysis of socio-psychological factors in patients with recurrent spontaneous abortion

Objective To explore the socio-psychological risk factors of patients with recurrent spontaneous abortion(RSA)and provide scientific evidence for their clinical intervention and treatment.Methods A total of 659 women of childbearing age who were admitted to Lanzhou University Second Hospital from February 2018 to February 2019 were selected for case-control study.The socio-psychological data of the subjects were collected through questionnaire survey.Data were analyzed in 216 patients with RSA(two or more spontaneous abortions)and 224 patients with non一RSA(no spontaneous abortion,and at least one normal pregnancy and successful delivery)from February 2018 to October 2018.A logistic regression model(LRM)was established and assessed the predictive value of the model by receiver operating characteristic curve(ROC).Validation was performed in patients admitted from November 2018 to February 2019.Results LRM includes age,education level,occupation,makeup,exposure to kitchen fumes,sleep time at night,the frequency of couples' sexual life,interpersonal relationships,and the presence or absence of depression.The total accuracy rate was 65.2%,the sensitivity was 69.4%,and the specificity was 61.2%.Subsequently,LRM was verified with an overall accuracy o£74.4%.Conclusion LRM is a valuable model for predicting psychosocial risk factors in RSA patients.
