摘    要:目的观察TACE联合射频消融(RFA)治疗累及第二肝门区肝细胞癌(HCC)的效果。方法收集41例HCC患者,共44个肿瘤累及第二肝门区,于TACE后行RFA治疗。采用Kaplan-Meier法绘制生存曲线,统计患者中位总生存期(OS)及中位无进展生存期(PFS)。结果第二肝门区HCC完全消融率为81.82%(36/44)。消融后17例出现疼痛、10例一过性肝功能损伤、9例发热、2例气胸、1例胸腔积液、1例出血,均经对症处理后痊愈;无死亡病例。41例患者术后中位OS为28个月,中位PFS为10个月。结论 TACE联合RFA可有效治疗累及第二肝门区的HCC,且并发症少,患者预后较好。

Abstract:Objective To explore the efficacy of TACE combined with radiofrequency ablation (RFA) for treatment of hepatocellular carcinoma (HCC) involved the second hepatic hilar region. Methods Forty-one patients with HCC involved the second hepatic hilar region were included. There were 44 tumors involved the second hepatic hilar region among 41 patients. All patients underwent RFA after TACE. The Kaplan-Meier method was used to draw survival curve, and the median overall survival (OS) and median progression-free survival (PFS) were calculated. Results The complete ablation rate of tumors involved the second hepatic hilar region was 81.82% (36/44). After RFA, pain occurred in 17 cases, transient liver function injury in 10 cases, fever in 9 cases, pneumothorax in 2 cases, pleural effusion in 1 case and bleeding in 1 case, but all were cured after symptomatic treatment. There was no treatment related death. The median OS was 28 months, and the median PFS was 10 months in all patients. Conclusion TACE combined with RFA can effectively treat HCC involved the second hepatic hilar region with few complications and relatively good prognosis.
