非酒精性脂肪肝患者肾动脉阻力指数的超声观察

摘    要:目的评价非酒精性脂肪肝(NAFLD)患者肾内动脉阻力指数的变化。方法采用二维及彩色多普勒超声(CDFI)在斜冠状面检测36例健康成人和36例非酒精性脂肪肝患者双侧肾脏,分别在近肾门部取双肾段动脉与近皮质部取弓形动脉频谱,比较非酒精性脂肪肝患者与健康成人肾内动脉阻力指数的差异。结果在健康成人,左右肾长度、左右肾间同区域肾内阻力指数均无显著性差别(P〉0.05),而在非酒精性脂肪肝患者,左肾长度显著大于右肾,较健康成人显著增长;与健康成人比较,非酒精性脂肪肝病人右肾段动脉及弓形动脉RI无显著性差别,而左肾动脉阻力指数有显著性降低(P〈0.01或P〈0.05)。结论非酒精性脂肪肝患者左肾内动脉阻力指数较健康成人降低。

Abstract:Objective The aim of this study was to investigate the intrarenal resistance index (RI) in patients with nonalcoholic fatty liver disease (NAFLD). Methods Thirty-six healthy adults and thirty-six patients with NAFLD underwent a conventional abdominal ultrasound and color Doppler flower imaging (CDFI) examination. CDFI signals were taken from segmental arteries near hilum and arcuate arteries of the cortex in the right and left kidneys,respectively. The difference of RI in segmental arteries and arcuate arteries were statistically evaluated. Results In healthy adults,the length of right and left kidney and intrarenal RI of two kidney had no significant difference (P〉0.05). Compared with healthy aduhs,patietns with NAFLD had a increased length of left kidney (P〈0.01) and a decreased RI in left segmental arteries (P〈0.01) and arcuate arteries (P〈0.05). Conclusion The intrarenal RI in patients with NAFLD was lower than that in healthy adults.
