腰椎椎体后缘软骨结节的X线与CT诊断

目的提高对椎体后缘软骨结节的认识。方法对38例椎体后缘软骨结节临床特征和X线、CT表现进行了综合分析。结果38例均以腰腿痛为主诉就诊,部分伴有间歇性跛行,X线腰椎侧位片见病变椎体(以腰4、5椎体为主)后下(或上)角有骨缺损及该区有游离骨块突入椎管内,CT主要特征表现为软骨结节,椎体后缘骨质缺损区和凸向椎管内的骨突环碎片,可伴有椎间盘突出。结论CT扫描对椎体后缘软骨结节有特征性表现,具有可靠的诊断价值。

Diagnosis of lumbar posterior marginal cartilaginous node by X-ray and CT

Objective To improve the knowledge of lumbar posterior marginal cartilaginous node. Methods The clinical manifestation and X-ray or CT findings of lumbar posterior marginal cartilaginous nods of 38 cases were analyzed synthetically. Results Thirty-eighty cases all went to receive medical treatment because of lumbago, leg pain, part of then combined with intermittent claudication. The lateral projection by X-ray examination showed that there were defects of posterior edge of involved vertebrae （mainly involving lumbar vertebrae 4,5） and free bony fragments sticked into the spinal canal. The typical CT findings included cartilaginous node, defect in the posterior margin and protruding follicle ring fragment, some of them could be with slipped disk. Conclusion CT examination can show main characteristic defects of lumbar posterior marginal cartilaginous node and provide valuable diagnosis.
