精子成熟障碍对孕早期自发性流产与胚胎停育的影响

摘    要:目的:研究精子不成熟度实验诊断指标在女方孕早期自发性流产、胚胎停育男性病因分析中的价值。方法:应用非匹配的病例对照研究方法,调查病例组(孕早期有自发性流产和/或胚胎停止发育史妇女的配偶,161例)与对照组(正常生育男性98例)精子-透明质酸未结合率、精子核蛋白不成熟率、精子DNA碎片率、精子形态和精液常规等指标的水平;二分类Logistic回归分析筛选致病因素,ROC曲线分析各项指标的诊断效率。结果:精子-透明质酸未结合率(OR=1.028,95%CI:1.007 ̄1.048;ROC曲线下面积为0.642)和精子核蛋白不成熟率(OR=1.040,95%CI:1.012 ̄1.069;ROC曲线下面积为0.649),是导致女方孕早期自发性流产、胚胎停育的独立因素;两者联合使用可进一步提高诊断效率(ROC曲线下面积为0.739,灵敏度0.679,特异性0.796,阳性似然比3.33,阴性似然比0.40)。结论:精子成熟障碍是导致女方孕早期自发性流产、胚胎停育的男性危险因素,精子-透明质酸未结合率和精子核蛋白不成熟率是有效的病因诊断指标。
