肿瘤样脑梗塞的诊治

目的探讨肿瘤样脑梗塞的诊断及鉴别诊断。方法回顾性分析6例术前诊断为脑肿瘤，术后经病理证实为脑梗塞病例的临床表现及影像学特点、诊治经过及病程转归。结果6例均经手术治疗，术后病理证实为脑梗塞。术前诊断：4例为胶质瘤，1例为转移瘤，1例为胆脂瘤。5例治愈，1例轻残。结论正确的鉴别有助于脑梗塞患者的明确诊断而免于不必要的手术，对于占位效应明显或合并出血、感染的病人应手术治疗。动态观察可避免误诊。

Diagnosis and treatment of tumor-like cerebral infarction

Objective To discuss the diagnosis and differential diagnosis of tumor-like cerebral infarction. Methods The clinical data of 6 patients, including their clinical manifestations, imaging characteristics, diagnosis and treatment, final outcome of their diseases, were retrospectively analyzed. All these patients were diagnosed as brain tumor at first time, but finally were proved to be cerebral infarction after operation. Results All the 6 patients were operated and verified as cerebral infarction via postoperative pathology. Preoperative diagnosis showed 4 were glioma, 1 metastatic tumor, and 1 cholesteatoma. After operation, 5 were cured and 1 in mild disability. Conclusion Those patients suffering from cerebral infarction unfortunately demonstrate some manifestations like brain tumor. So an accurate identification is helpful to protect them from unnecessary operation. But under such conditions as massive occupancy complicated with hemorrhage or severe intracranial infection, the patients must be operated on. Besides, dynamic observation can avoid misdiagnosis.
