Abstract:Objective To observe the extrahepatic origin, course, diameter, branch and variation of the proper hepatic artery with 256-slice spiral CT angiography (CTA), so as to provide reference for the diagnosis and treatment of upper abdominal diseases. Methods A total of 80 patients who underwent abdominal CTA in the Imaging Department of the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from Jun. 2017 to Jun. 2019 were collected. The variation of proper hepatic artery and the branches were counted and analyzed, and the related data were measured. Results The extrahepatic origin variation of proper hepatic artery and the branches was found in 15 cases (18.75%). The length of proper hepatic artery was (45.44±22.76)mm, and the internal diameter of the starting point was (2.66±1.68)mm. The length of left hepatic artery was (43.68±18.75)mm, and the internal diameter of the starting point was (3.23±1.09)mm; the length of right hepatic artery was (43.10± 18.50)mm, and the internal diameter of the starting point was (3.26±1.08)mm. The angle between left hepatic artery and right hepatic artery was (75.22±22.36)°. The length of the middle hepatic artery was (59.21±23.75)mm, and the internal diameter of the starting point was (1.99±0.71)mm. The angle between the middle hepatic artery and left hepatic artery was (84.81±39.94)°, and the angle between the middle hepatic artery and right hepatic artery was (57.31±17.28)°. Conclusion The extrahepatic origin of proper hepatic artery and the branches are complex and varied. Abdominal CTA can truly and accurately reflect the morphology in vivo. By measuring the length and diameter and deepening the understanding of variant hepatic artery, it will be helpful for the smooth implementation of upper abdominal surgery.
