外伤性迟发性颅内血肿的诊治分析并文献复习

目的探讨外伤性迟发性颅内血肿的诊治要点.方法回顾分析52例外伤性迟发性颅内血肿病例,其中外伤后24小时内确诊36例,72小时内确诊11例,1周内确诊5例.复习其诊疗过程及相关文献,分析外伤性迟发性颅内血肿的诊治要点.结果 1例确诊时已并发脑疝,33例行开颅手术,15例颅内血肿较少行保守治疗,3例因年龄大、体质差放弃手术.其中40例取得良好效果,中残6例,重残3例,死亡3例.结论认识迟发性颅内血肿的早期临床表现及CT特点,对此病的救治有重要意义.

Analysis of the diagnosis and treatment in traumatic tardive intracranial hematoma and literatures reviewed

Objective To explore the major points of diagnosis and treatment in traumatic tardive intracranial hematoma. Methods Review and analyse 52 patients who had traumatic tardive intracranial hematoma.of 52 cases, 36 patients were confirmed in 24 hours after injury.11 patients were confirmed in 72 hours and 5 patients were in 1 week. we reviewed the process of diagnosis and treament in these cases and relative literatures,analyzing the main points of the disease. Results 1 case has cerebral hernia when confirmed. 33 cases performed skull operation.15 cases performed expectant treatment because of little haematoma.3 cases gived up further operation because of old age and bad condition. Of the 52 cases, 40 cases received well results. 6 cases got disability and 3 cases got severe disability.3 patients died during hospitalization. Conclusion It have important meaning to recognize the early clinical manifestation and CT characteristic for the successful treatment of traumatic tardive intracranial hematoma.
