幽门螺杆菌对呋喃唑酮的耐药状况及耐药机制的研究

摘    要:目的了解福建省福州地区上消化道疾病患者幽门螺杆菌(Helicobacter pylori,H.pylori)对呋喃唑酮的耐药情况,探讨H.pylori对呋喃唑酮的耐药机制。方法分离培养获得63株H.pylori,用琼脂稀释法进行体外抗生素敏感试验,检测H.pylori对呋喃唑酮的耐药率,挑选出耐药菌株,提取细菌DNA,PCR扩增porD、oorD基因,PCR产物扩增后测序,比较耐药菌株和标准菌株的基因序列。结果共检测出4株耐药菌株,福州地区H.pylori对呋喃唑酮的耐药率为6.35%,呋喃唑酮耐药与年龄无关。PCR从H.py—lori基因组DNA中扩增出592bp的H.pyloriporD基因和465bp的H.pylorioorD基因,与标准菌株的基因序列比较,耐药H.pylori的porD基因和oorD基因出现点突变。结论福州地区H.pylori对呋喃唑酮的耐药率较低,porD和oorD基因突变可能与H.pylori对呋喃唑酮的耐药有关。

Abstract:Objective To investigate the prevalence and mechanism of Helicobacter pylori (H. pylori) resistance to furazolidone in Fuzhou area of Fujian province. Methods Sixty-three clinical isolates were cultured. The susceptibili- ties of the strains to furazolidone were tested by agar dilution test. The H. pylori resistant strains to furazolidone were se- lected. The DNA of selected H. pylori strains was extracted and PCR was performed to amplify porD and oorD genes. The PCR amplification products were sequenced after cloning. Then, the sequences were compared with standard strain. Results Four H. pylori resistance strains were detected in sixty-three isolates of H. priori. The rate of H. pylori resist- ance was 6. 35% , and the resistance showed no correlation with the patients age. A oorD gene DNA fragment with length of 465 bp and a porD gene DNA fragment with length of 592 bp were amplified from the genomic DNA of resistant H. pylori strains and point mutation in porD and oorD gene of the resistance isolates was found. Conclusion The results indicate that the prevalence of H. pylori resistant to furazolidone in Fuzhou area is relatively low. Genetic mutation in porD and oorD gene may contribute to H. priori resistance to furazolidone.
