开胸术后两种镇痛方法的疗效观察

目的观察开胸术后两种不同止痛方法的应用疗效。方法122例患者随机分为两组,每组61例。1组患者采用肋间神经压榨术,2组患者行硬膜外置管,术后接镇痛泵。结果1组镇痛效果明显高于2组,差异均有显著性（P〈0.01）。不良反应及并发症发生率,两组比较P〈0.01,1组哌替啶用量较2组明显减少,P〈0.01。结论肋间神经压榨术能减轻开胸术后患者切口疼痛。并且安全可靠,能减少镇痛药用量,并可减少术后并发症的发生。

Observation on the therapeutic effect of two analgesic methods after thoracotomy

Objective To observe the effect of different two analgesic methods after thoracotomy.Methods A random prospective study was performed on 122 patients after thoractomy.Patients were divided into 2 groups（each group was 61 patients）：group 1 received intercostals nerve crush;group 2 used patient controlled epidural analgesia;Effectiveness of different analgesia methods and their influence on postoperative complications were compared.Results Analgesia effects in group 1 were much better than group 2（P〈0.01）;dosage of dolantin in group 1 was much less than group 2（P〈0.01）, postoperative complications in group 1 were much less than group 2（P〈0.01）.Conclusion Intercostal nerves crush can effectively relieve postoperative chest pain and reduce the dosage of narcotics,the occurrence of complications is an effective and reliable way to prevent intense chest pain after thoracotomy.
