完全性肺静脉畸形连接的超声心动图诊断价值

目的探讨超声心动图对完全性肺静脉畸形连接(total anomalous pulmonary venous connection TAPVC)的扫查方法及特征性表现.方法应用彩色多普勒超声或造影检查14例TAPVC.结果 14例TAPVC中,心上型5例,心内型6例,心下型1例,混合型2例.除心下型1例(7.1%)漏诊外,诊断准确率为78.6%(11/14),基本符合率(混合型)14.3%(2/14),总符合率为92.9%.结论应用超声心动图可确定本病的诊断及分型,判断有无肺静脉狭窄、肺动脉高压及预后.对混合型和较少见的心上型TAPVC的诊断准确率有待于进一步提高.

Diagnosis of Total Anomalous Pulmonary Venous Connection by Echocardiography and Colour Dppler Flow Imaging

Objective To assess echocardiographic detecticg technique and imaging characters of total anomalous pulmonary venous connection(TAPVC).Methods Fourteen patients with TAPVC were studied via colour Doppler flow imaging(CDFI)or contrast echocardiography.Results Fourteen patients included 5 of supracar diac type,6 of cardiac type,1 of infracardiac type and 2 of mixed type.Echocardiographic diagnostic accuracy was 78.5%(11/14).The total coincidence rate was 92.8%(11/14).Conclusions Echocardiography can be used in diagnosis and classification of TAPVC,in judgement of pulmonauy venous drainage stenosis,pulmonary artery hypertension(PH)and in prediction of the prognosis.It is very difficult to diagnose supracardiac type and mixed type of TAPVC.
