COOK球囊与催产素在高危妊娠孕妇引产中的效果观察

摘    要:目的探讨COOK球囊用于高危妊娠孕妇引产中的效果。方法选取新乡市某医院产科2015年1月―2019年6月收治的140例高危妊娠孕妇为研究对象。采用随机数字表法,将孕妇随机分为观察组与对照组,每组70例。对照组孕妇使用催产素引产。观察组孕妇采取COOK球囊引产。比较2组孕妇放置COOK球囊或应用催产素前后12 h宫颈Bishop评分;比较2组孕妇第一产程时长、总产程时长、产钳助产率、剖宫产率、自然分娩率;比较2组孕妇宫颈裂伤、产后发热、产后出血、阴道血肿、宫缩过强、胃肠道症状等并发症发生情况。结果干预后12 h,观察组孕妇Bishop评分为(6.98±1.09)分,高于对照组的(6.16±1.02)分,差异有统计学意义(P<0.05)。观察组孕妇总产程时长为(13.49±2.30)h,短于对照组的(16.87±2.51)h,差异有统计学意义(P<0.05)。2组孕妇产钳助产率、转剖宫产率及自然分娩率比较,差异无统计学意义(P>0.05)。观察组孕妇并发症发生率为12.86%,低于对照组的41.43%,差异有统计学意义(P<0.05)。结论于在高危妊娠孕妇的引产中应用COOK球囊,可有效促进宫颈成熟,缩短引产时间,同时还可避免药物不良反应,安全性高,值得临床推广应用。

Abstract:Objective To investigate the effectiveness of COOK balloon on induced laboring in high-risk pregnant women.Methods A total of 140 high-risk pregnant women in a Xinxiang hospital from January 2015 to June 2019 were selected as study objects and divided into the observation group(n=70)and the control group(n=70)according to the random digits table.The oxytocin was used in the control group,while the COOK balloon was used in the observation group.Cervical Bishop scores before and after 12 h of COOK balloon or oxytocin in two groups were compared;laboring period,total laboring stage,forceps delivery rate,uterine-incision delivery rate and spontaneous delivery rate in two groups were compared;complications of cervical laceration,postpartum fever,postpartum hemorrhage,vaginal hematoma,uterine over-efficiency and gastrointestinal symptom in two groups were compared.Results After intervention of 12 h,Bishop score in the observation group was(6.98±1.09),which was higher than that of(6.16±1.02)in the control group,with statistically significant differences(P<0.05).Laboring period in the observation group was(13.49±2.30)h,which was shorter than that of(16.87±2.51)h,with statistically significant differences(P<0.05).Comparison of forceps delivery rate,uterine-incision delivery rate and spontaneous delivery rate in two groups had no statistically significant differences(P>0.05).Complications in the observation group was 12.86%,which was lower than that of 41.43%in the control group,with statistically significant differences(P<0.05).Conclusion Application of COOK balloon in high-risk pregnant women can effectively promote the cervical ripening,shorten the induction time and avoid the adverse drug reactions,which has high safety and is worth of clinical promotion.
