子宫动脉栓塞治疗子宫肌瘤疗效评价

目的:评价子宫动脉栓塞(UAE)治疗症状性子宫肌瘤(UF)的临床疗效和副反应。方法:对40例症状性子宫肌瘤患者进行超选择性双侧子宫动脉用PVA微粒栓塞,随访12-24个月,复查观察术后症状缓解情况和子宫肌瘤缩小程度。结果:除2例仅完成单侧操作外,其余均治疗成功,完整随访复查术后瘤块缩小30%-70%。所有病例,月经恢复正常,贫血病例,血红蛋白升至正常范围。副反应主要为术后下腹部疼痛(12/40),无严重并发症发生。结论:选择性子宫动脉栓塞为非手术治疗症状性子宫肌瘤的较好疗法。

Uterine Artery Embolization for Treatment of Uterine Fibroids

Objective: To evaluate the effect and side effects of uterine arterial embolization(UAE) as a treatment of symptomatic uterine fibroids (UF). Methods: A total of 27 cases with clinic symptoms caused by uterine fibroids underwent UAE. The embolization of bilateral uterine arteries was performed with a single catheter via femoral artery approach with an injection of PVA (355-500 μm). The improvement of symptoms and tumors size changes were followed up in 12-24 months after the procedure. Results: All but two cases were successfully treated bilaterally, tumor size was decreased by 30%-70%, and menses returned to regular in all patients. In cases with anemia, the hemoglobin concentration increased to normal level. The main side effects were hypogastic pain (12/40). No serious complications occurred. Conclusion: Uterine arterial embolization is a good non-surgical therapy for symptomatic uterine fibroids with mild side effects.
