食管癌三维适形放射治疗疗效观察

目的:探讨三维适形放射治疗(3DCRT)食管癌的近期疗效及放射治疗反应。方法:75例经病理证实的食管癌患者,分为常规放疗组(CF组)和三维适形放射治疗组(3DCRT组)。其中CF组40例,3DCRT组35例。CF组:2GY/次,5次/周,总剂量60-66Gy.7周内完成;3DCRT组:先常规放疗20GY,方法同CF组.然后改用三维适形放疗,2Gy/次,5次/周,总剂量60-66Gy,7周内完成。以放疗结束时临床症状及放疗结束后3个月的食管钡餐片作为评价依据。结果:CF组和3DCRT组的有效率(CR+PR)分别为85%、100N,两组比较差异有统计学意义(x2=3.85,P<0.05)。两组比较放射性食管炎发生率差异无统计学意义(x2=3.40,P>0.05),两组比较放射性气管炎发生率差异无统计学意义(X2=3.24,P>0.05)。结论:三维适形放射治疗能明显提高食管癌的近期疗效,放射性毒性可耐受。

Three-dimensional conformal radiotherapy for esophageal carcinoma: preliminary report of 35 patients

Objective: To evaluate the result and side effects in three dimensional conformal radiation therapy (3DCRT) for esophageal carcinoma. Methods: From 75 patients with pathologically confirmed esophageal carcinoma were divided into conventional radiotherapy (CF) and three dimensional conformal radiotherapy (3DCRT) groups, with 35 patients in 3DCRT group and 40 patients in CF group. The patients in CF group received conventional radiotherapy in 2. 0 Gy/f, 5 fractions a week to a total dose of 60-66 Gy in 7 weeks. The patients in 3DCRT group were first treated by the same fractionation in CF group to the dose of 20 Gy, and then treated by 3DCRT in 2. 0 Gy/f,5 fractions a week to the total dose of 60-66 Gy in 7 weeks. Results:Effection rate(CR+PR)were 85%and 100%in CF group and in 3DCRT group respectively (x2=3. 85, P <0. 05). Between the two groups, there was no significant difference in radiation esophagitis(x2=3. 40, P >0. 05) , and there was no significant difference in radiation bronchitis(x2 = 3. 24, P > 0.05). Conclusions; Lately effection rate of patients with esophageal carcinoma treated by three dimensional conformal radiotherapy is superior to conventional radiotherapy, and radiation toxicities could be endure.
