角膜内皮炎17例误诊分析

目的：分析角膜内皮炎误诊的原因。方法：回顾性分析17例被误诊的角膜内皮炎的发病诱因、临床表现、分型及治疗。结果：误诊为角结膜炎3例，误诊为急性闭角型青光眼2例，误诊为白内障术后炎性反应1例，误诊为盘状角膜基质炎4例，误诊为虹膜睫状体炎3例，误诊为青光眼睫状体炎综合征4例。所有病例经治疗后痊愈，其中2例复发。误诊的主要原因是对本病的认识不足，缺乏细致的检查。结论：加强对角膜内皮炎的认识，正确掌握不同类型角膜内皮炎临床特征和鉴别诊断的关键点，对患者仔细检查，可有效降低误诊率。

Analysis on misdiagnosis of corneal endotheliitis in 17 patients

Objective To analyze the causes of misdiagnosis for corneal endotheliitis.Methods Seventeen misdiagnosed cases of corneal endotheliitis were analyzed retrospectively,including predisposing factors,clinical manifestations,clinical classification and treatment.Results Of 17 cases,3 cases were misdiagnosed as keratoconjunctivitis,2 as acute close-angle glaucoma,1 as inflammatory reaction after cataract extracapsular extraction and IOL implantation,4 as disciform stromal keratitis,3 as iridocyclitis and 4 as glaucomatocyclitic crisis.All the 17 cases were cured through treatment.The main causes of misdiagnosis consisted of insufficient knowledge on it and lack of careful examination.Conclusion Pay more attention to the knowledge of corneal endotheliitis,properly grasping its features and key points for differential diagnosis and detailed examination will benefit to decrease the misdiagnosis.
