小切口翼状胬肉切除术与传统翼状胬肉切除术疗效的临床观察

目的观察小切口翼状胬肉切除联合自体结膜瓣角膜缘干细胞移植术(以下简称小切口组)和传统翼状胬肉切除联合自体结膜瓣角膜缘干细胞移植术(以下简称传统切口组)治疗初发性翼状胬肉的疗效。方法收集哈尔滨爱尔眼科医院2011年8月～2011年10月在角膜眼表科就诊手术治疗的翼状胬肉患者60例(100只眼),其中30例(52只眼)为小切口手术组,另30例(48只眼)为传统手术组,小切口手术组在翼状胬肉切除术中结膜切口大小小于3mm×3mm,传统手术组切口大小大于5mm×5mm。比较2组患者的手术时间,恢复时间,患者不适感,复发率,并发症等。结果半年后,小切口组0眼复发,复发率为0%。传统切口组2眼复发,复发率为4.1%,两组比较无统计学意义(χ2=2.208,P>0.05)。手术时间:小切口组短于传统切口组(t=23.7,P<0.05)。恢复时间小切口组短于传统切口组(t=7.89,P<0.05)。2组手术复发率差异无显著性(χ2=2.208,P>0.05)。结论 2种手术方式复发率无差异性,但是小切口组手术时间短,出血少,恢复时间短,不适感轻,明显提高手术质量,减轻患者的痛苦。

Methods collect Harbin Department of ophthalmology hospital in 2011 August -2011 year in October in the cornea clinic operation in treatment of pterygium in 60 cases （ 100 eyes）, 30 cases （ 52 eyes ） for small in- cision operation group, another 30 cases （ 48 eyes ） for the traditional operation group, Small incision operation group in pteryglum excision of conjunctival incision size less than 3mm × 3mm, The traditional group operation incision size greater than 5mm ×5mm. Comparing the 2 groups of patients with operation time, recovery time, patient discomfort, recurrence rate, complication. Results after half a year, the small incision group 0 cases of recurrence, relapse rate of 0%. Tradi- tional incision group 2 cases of recurrence, relapse rate of 4. 1%. Operation time： small incision group was shorter than the conventional incision group. Recovery time of small incision group was shorter than the conventional incision group. 2 groups of operation relapse rate had no significant difference （ P 〉 0.05 ）. Complications ： 2 groups had no serious compli- cations. Conclusions the 2 kinds of operation mode of recurrence rate showed no difference, but the small incision group short operation time, less bleeding, shorter recovery time, discomfort light, significantly improve the quality of operation, reduce the pain of patients.
