亲属活体供小肠移植术后并发症的监测

目的 探讨采用内镜与病理检查监测亲属活体供小肠移植术后并发症的意义。方法 2005年1月11日为1例诊断为小肠切除术后短肠综合征的患者进行亲属活体供小肠移植术，供者为受者的母亲。术后采用肠镜和病理检查监测受者移植术后并发症。结果 小肠移植术后37d，内镜监测和病理活检发现急性排斥反应，术后65d时病理活检发现巨细胞病毒感染。给予激素冲击、OKT3抗排斥治疗，结合更昔洛韦、阿昔洛韦抗病毒治疗，患者病情得到控制，随访至今已存活16个月。结论 采用肠镜监测和病理活检可以及时发现小肠移植术后隐匿的病情，提高小肠移植患者的存活率。

Monitoring of complications following living-related small bowel transplantation

Objective Investigate endoscopic monitoring and pathological evaluation of intestinal graft in living-related small bowel transplantation. Methods Methods A case of small intestinal transplantation with mother donor was successfully performed in the First Affiliated Hospital of Sun Yatsen University in Nov. 11, 2005. The complications of intestinal graft were monitored with endoscope regularly after transplantation. Results The operation of donor and recipient were performed successfully. Acute cellular rejection was diagnosed with pathological methods by endoscopic monitoring 37 days after transplantation. CMV infection was found in pathological examination 65 days after transplantation. The complications were successfully controlled by steroid impact and therapy of anti-virus. Now the recipient survived for 18 months after transplantation. Conclusions Cellular rejection and CMV infection are the common complications of small bowel transplantation. Post-operational monitoring and endoscopic monitoring was helpful to find and control the complications.
