老年大肠复合性息肉的临床分析

目的 探讨老年大肠复合性息肉患者的临床特点及其与癌变的关系。方法 对336例老年大肠息肉患者结肠镜检查发现复合性息肉86例，分别活检。结果 复合性息肉发生率为25．59％，随年龄增长检出率增加；多累及2个以上的肠段；分布多见于直肠、乙状结肠；以炎性息肉和腺瘤性息肉并存多见。腺瘤发生率为46．07％，高于单发性及多发性息肉，有显著性差异(P＜0．01)。复合性息肉的癌变率为12．79％，与单发及多发性息肉比较无显著性差异。结论 对老年复合性息肉要重视，对多发息肉要分别做病理检查，提高诊断率。

Clinic analysis on compound large bowel polyps of elderly people

Objective To understand the characteristics of compound large bowel polyps in aged people,and the relationship between the ployps and malignancy is studied.Methods A total of 336 elderly patients with colonic polyps diagnosed by endoscopy were analyzed.Among them,86 cases with compound large bowel polyps were found by endoscopy and pathology.Results The incidence of compound large bowel polyps in aged patients were 25.59% which showed gradual increas in trend with the age.The compound large bowel polyps were usually observed in rectum and sigmoid colon.Inflamatory and adenomatous polyps coexist in different bowel segments are more frequently encountered.Among the compound large bowel polyps,adenomatous polyps were 46.07%,being significantly higher than single polyps and multiple polyps.The malignant frequency of compound polyps were 12.79%,being not different from that of multiple and single polyps.Conclusions More attentions should be paid to the compound large bowel polyps in aged people.Multiple polyps should be examined with endoscopy and pathology respectively so that the diagnosis of the compound large bowel polyps can be picked out.
