围手术期预防性使用抗菌药监测

目的：了解我院围手术期预防性使用抗菌药物的情况并评价其合理性。方法：采用回顾性分析方法，随机抽取我院120份手术病历，统计围手术期预防性使用抗菌药情况，评价其合理性。结果：120份手术病历中，预防性使用抗菌药的有110例，占总体的91.6%。在这110例预防性使用抗菌药的患者中，抗菌药费为1000元以下的81例，占73.7%，说明大部分围手术期预防性使用的抗菌药物价格相对较低廉，有利于减轻患者的经济负担。预防性使用的抗菌药共涉及31个药品，其中头孢菌素类最多，使用频次排名前五位的分别是头孢呋辛、头孢匹胺、甲硝唑、头孢噻肟和帕珠沙星。64.5%病历术前用药时间合理，92.7%病历术中用药合理，而45.5%病历术后用药时间过长。结论：我院围手术期预防性使用抗菌药尚存在某些不合理之处，应加强监测和管理。

Monitoring of Preventive Antibacterials Used in the Perioperative Period

Objective： To investigate preventive antibacterials used in the perioperative period in our hospital and evaluate its rationality. Methods： Using retrospective analysis methods, the use of preventive antibacterials in perioperative period in a random sample of 120 surgical cases in our hospital were analyzed and the rationality was evaluated. Results： There were 110 cases of preventive use of antibacterials for operation, accounting for 91.6%. Among these 110 cases, there were 81 cases that the expense of antibacterials was under 1 000 yuan, accounting for 73.7%. This indicated that the expense of most of perioperative preventive antibacterials was low, which was in favor of reducing the patients＇ financial burden a lot. There were 31 drugs involved in the current study, among of them, cephalosporins were used mostly. The high frequency usage of antibacterials were cefuroxime, cefpiramide, metronidazole, cefotaxime and pazufloxacin respectively. Regarding medication time of preoperative and intraoperative medication, there were 64.5% and 92.7% reasonable cases, respectively. But there were 45.5% cases in which postoperative medication time was too long. Conclusion： There were some irrational use in the perioperative preventive use of antibacterials in our hospital. Monitoring and management should be strengthened.
