高血压病患者血压昼夜节律与左心室肥厚及心肌缺血之间的关系探讨

摘    要:目的,探讨高血压病患者血压昼夜节律模式与左心室肥厚及心肌缺血之间的关系。方法:根据夜间血压是否下降明显将64例高血压病患者分为两组:昼夜节律正常组(杓型30例)和异常组(非构型34例),观察和比较组间动态血压参数,左心室生理指数和心肌缺血的发生率。结果:两组间24h平均舒张压和舒张压负荷,白昼平均收缩压,收缩压负荷,舒张压及舒张压负荷无显著性差异(P>0.05),非构型组24h平均收缩压和收缩压负荷,夜间平均收缩压,收缩压负荷,舒张压和舒张压负荷较榴型组显著增高(P<0.05-0.01),左心室重量指数显著增加(P<0.01),心肌缺血发生率显著增高(P<0.01),结论:高血压病昼夜节律减弱和消失患者较节律正常患者左心室肥厚更重,心肌缺血发生率更高。

Abstract:Objective To study the relationships between circadian profiles of blood pressure(BP), left ventricular hypertrophy(LVH) and myocardial ischemia(MI) in patients with essential hypertension.Methods We investigated the data of 24h BP, left ventricular mass index(LVHI) and the occurrence of MI in 64 cases with essential hypertension, which were divided into two groups:the normal groups("dippers" n=30) and the abnormal group("non dippers" n=34).Results The diastolic blood pressure(DBP) and DBP load during 24h and the systolic blood pressure(SBP),SBP load, DBP and DBP load during day time were no difference between the two groups; the SBP and SBP load during 24h and SBP, SBP load, DBP and DBP load during night time were higher in "non dippers" than in "dippers";the LVHI and the occurrence of MI were also higher in "non dippers".Conclusion The LVH is more serious in cases with silent circadian rhythm than in cases with normal circadian rhythm, the occurrence of MI is also higher in cases with silent circadian rhythm.
