血清造血生长因子在宫颈癌早期诊断中的价值

摘    要:目的 检测宫颈癌及癌前病变患者血清中造血生长因子(HGFs)的表达水平,评估HGFs在宫颈癌早期诊断中的应用价值.方法 选择宫颈癌患者49例、宫颈上皮内瘤变(CIN)患者60例、健康体检者40例为研究对象,采取血清标本,酶联免疫法检测HGFs表达水平[包括干细胞因子(SCF)、粒细胞巨噬细胞刺激因子(GM-CSF)、粒细胞集落刺激因子(G-CSF)和巨噬细胞集落刺激因子(M-CSF)],化学发光微粒免疫分析法检测宫颈癌肿瘤标记物鳞状细胞癌抗原(SCC-Ag)表达,对检测结果 进行对比分析.结果GM-CSF、G-CSF和M-CSF在宫颈癌及CIN患者中表达水平显著高于健康体检者(P<0.05),GM-CSF和M-CSF在宫颈癌患者的表达水平显著高于CIN患者(P<0.05),SCC-Ag在宫颈癌患者的表达水平显著高于CIN患者和健康体检者(P<0.05).M-CSF、GM-CSF、G-CSF、SCC-Ag诊断宫颈癌的特异度均较高,其中M-CSF诊断的敏感度、阳性预测值和阴性预测值均高于SCC-Ag,M-CSF与SCC-Ag组合后诊断水平高于单独应用SCC-Ag.结论 对HGFs的深入研究可能为寻找新的宫颈癌肿瘤标记物开辟新的途径,尤其是M-CSF具有作为一个良好的宫颈癌和CIN标记物的潜能,可作为早期诊断宫颈癌及CIN的可能因子.

Abstract:Objective To investigate the plasma levels of hematopoietic growth factors ( HGFs) in patients with cervical cancer or cervical intraepithelial neoplasia (CIN), thus to evaluate its application in early diagnosis of cervical cancer.Methods The plasma levels of HGFs ( SCF, GM-CSF, G-CSF and M-CSF) were determined by immu-noenzyme assay (ELISA), and SCC-Ag by chemiluminescent microparticle immunoassay .Results Significantly higher GM-CSF, G-CSF and M -CSF were observed in patients with cervical cancer and CIN than healthy controls ( P<0.05).Significant higher GM -CSF and M-CSF were observed in patients with cervical cancer than those in patient with CIN (P<0.05).The diagnostic specificities GM -CSF, G-CSF, M-CSF and SCC-Ag were high for cervical cancer . The diagnostic sensitivity and the positive predictive value , and negative predictive value of M -CSF were higher than an-tigen SCCAg.Conclusion Further study of HGFs suggests new biomarker for cervical cancer .M-CSF can applied as a biomarker for early diagnosis of cervical cancer and CIN .
