无菌吸痰管吸痰与纤维支气管镜肺泡灌洗在机械通气患者下呼吸道感染病原学诊断中的应用比较

目的：比较无菌吸痰管吸痰与纤维支气管镜肺泡灌洗采样培养在机械通气患者下呼吸道感染病原学诊断中的价值。方法：64例机械通气合并下呼吸道感染患者，均先后使用无菌吸痰管吸取下呼吸道分泌物和纤维支气管镜肺泡灌洗两种方法进行采样，并进行病原学分析和诊断试验的评价。结果：无菌吸痰管与支气管肺泡灌洗采样培养的阳性率分别为71.88%和78.13%(P>0.05)，两种方法诊断一致率达81.25%。结论：无菌吸痰管采样对于机械通气合并下呼吸道感染患者，不仅能获得准确的病原，帮助临床医生选择敏感抗生素，而且还是一种简单、安全和经济的病原采集方法，值得推广应用。

Diagnostic value of sterility sputum aspirating tube and bronchoalveolar lavage by bronchofibroscope in mechanically ventilated patients with lower respiratory tract infection

ObjectiveTo compare the diagnositic value of sterility sputum aspirating tube with bronchoalveolar lavage in mechanically ventilated patients with lower respiratory infection.MethodsSixty-four mechanically ventilated patients with lower respiratory infection were selected to collect respiratory passage secretion to analyze the etiology and to evaluate the diagnostic value by using sterility tube aspirate sputum and bronchoalveolar lavage.ResultsThe positive rate of sterility tube aspirate sputum and bronchoalveolar lavage was 71.88% and 78.13%, respectively, with no statistical difference between the 2 groups (P>0.05). The consistency rate was 81.25% for the 2 methods. ConclusionSterility sputum aspirating tube can not only acquire accurate pathogen, provide evidence to sellect sensitive antibiotics in clinical practice, but also is a simpleal practice, safe and economical method for patients with lower respiratory tract infection and mechanical ventilation.
