远端胃癌根治术(D4)的临床操作体会

目的:探讨远端胃癌D4术式的手术技巧,旨在减少术中出血、损伤及术后并发症。方法:将43例远端胃癌分为对照组(N1= 21)和改进组(N2=22),比较了手术时间、术中出血量、术中损伤、术后并发症等。对手术时间、术中出血量数据以(±s)表示,采用t检验,术中损伤、术后并发症采用卡方检验,评价了改进后的手术方法。结果:改进组在术中出血量较对照组显著减少,具有统计学意义(t=3.864,P<0.05),手术时间两组无显著差异,其他指标较对照组明显下降,但无统计学意义。结论:改进后的手术方法可有效减少术中出血、损伤及术后并发症。

Improvement in Operational Technique of Radical Excision(D4)for Distal Gastric Cancer

Objective:To study the operative technique of distal gastric cancer in D4 radical excision, and comph'cation after operation. Methods: Forty-three patients with distal gastric cancer were divided into control group(N1 = 21) and improved group(N2 = 22) for investigating and comparing the duration of operation, the magnitude of bleeding and injury during operation, and postoperative complications in both groups. The duration of operation and magnitude of bleeding were calculated by paired samples t - test and their values were produced by "x ± s". Injury and complications were calculated by x2 test. At last, the improved operative technique was evaluated.Results:Improved group was less than the control in bleeding magnitude,and the difference was stastically significant (t = 3.846, P < 0.05) . But there was little difference in operation duration between them. Improved group was less than the control in injury and complication,but there was no stastical difference, either. Conclusion: Improved operation technique may decrease bleeding magnitude,injury and complication effectively.
