膝关节损伤的MRI诊断及临床价值

摘    要:目的:研究膝关节损伤的MRI特点,诊断价值及临床意义。方法:回顾性分析100例经手术及临床证实的膝关节损伤MRI资料。结果:100例膝关节损伤患者中:(1)半月板损伤82例,其中Ⅰ度损伤33例,Ⅱ度损伤27例,半月板内异常信号未达关节面;Ⅲ度损伤22例,半月板内异常信号延伸至关节面,“桶柄状”撕裂11例,半月板移位变型12例,半月板囊肿13例;(2)前叉韧带撕裂伤28例,后叉韧带撕裂18例,均表现为低信号韧带中波浪状、斑片及团块状异常信号;(3)侧副韧带损伤21例;(4)关节积液84例;(5)骨折65例;骨髓挫伤84例。结论:MRI对膝关节损伤的诊断具有重要的临床意义,可为临床手术及治疗提供可靠的依据。

Abstract:Objective:To study the MRI characteristice,diagnostic value and its chinical significance of knee joint injury.Methods: 100 patients with knee jiint injury confirmed by operation and clinic were analysed retrospectively.Results:Of 100 patients with knee joint injury:(1)82 were meniscus injury,among them 33 were the first degree injury;27 were the second degree injury and the abnormal signal changein meniscus did not arrive at the articular surface;22 were the third degree injury and the abnormal signal change in meniscus al- ready arrived at the articular surface;11 exhibited laceration as"bucket handle shape";12 meniscus displacement and distortion,and 13 meniscal cyst.(2)28 were anterior cruciate ligament laceration and 18 posterior cruciat ligament laceration,both of them showed the MR findings of interleaved small signal intensity patchy with curvilinear or irregular shape.(3)21 were collateral ligament injury.(4)84 were joint hydrocele.(5)65 were fracture and 84 bone marrow injury.Conclusion:MRI plays an importnat role in the dignosis of knee joint in- jury,and is able to provide objective and detail foundations for correct therapeutic selection of orthopaedists.
