超声引导下穿刺活检在艾滋病患者合并颈部淋巴结肿大的临床应用

目的 探讨应用超声引导穿刺活检在艾滋病患者合并颈部淋巴结病变的临床应用价值。方法 回顾性分析201年 月—2017 年 9 月超声引导下穿刺颈部淋巴结肿大的29 例艾滋病患者的诊断率及并发症发生情况。结果 本组穿刺活检淋巴结大小为2.2~8.0 cm,平均(4.6±1.7) cm,取材满意率为93.1%（27/29）, 27 例取材满意的患者中,良性病变12例,包括结核8例,腮腺多形性腺瘤2例,淋巴结感染2例;恶性病变15例,包括卡波西肉瘤3例,淋巴瘤12例;并发症发生率6.9%（2/29）,2例发生穿刺后出血。结论 超声引导穿刺艾滋病患者颈部肿大淋巴结具有实时、创伤小、诊断性高、并发症少等优点,同时可减少医务人员职业暴露,具有一定的临床价值。

Clinical application of ultrasound-guided percutaneous biopsy in AIDS patients associated with cervical lymph node lesions

Objective To explore the clinical value of ultrasound guided puncture biopsy in AIDS patients associated with cervical lymph node lesions. Methods Clinical data (diagnostic rate and complication) about 29 AIDS patients with associated with cervical lymph node lesions who underwent ultrasound guided puncture biopsy from January 201to September 2017. Results The lymph node size ranged from 2.cm to 8 cm, average (4.6±1.7) cm, in this group, and the sampling satisfaction rat was 93.1% (27/29). 1patients were benign lesions among the 27 cases, including 8 cases of tuberculosis, cases of parotid pleomorphic adenoma and cases of lymph node infection; 15 patients were malignant lesions, including cases of Kaposi's sarcoma and 1cases of lymphoma. The incidence of complications was 6.9% (2/29). Two patients were occurred hemorrhage after biopsy. Conclusions Ultrasound guided puncture biopsy in AIDS patients associated with cervical lymph node lesions has the advantage of real-time, minimal invasiveness, high diagnostic rate and less complications, is thus worthy of application in clinical practice. Meantime, it can reduce the occupational exposures of the doctors.
