摘    要:目的 了解广东省江门市人乳头瘤病毒(HPV)的感染及其基因亚型分布情况,为该市今后防治人乳头瘤病毒感染和宫颈癌发病提供参考依据。方法 采用基因扩增技术及膜杂交原理,回顾性分析2015年1月—2017年12月在江门市中心医院就诊的31 523例女性生殖道HPV感染基因亚型筛查,比较不同基因亚型的流行病学特点。采用SPSS 22.0对数据进行统计学分析。结果 31 523例女性生殖道HPV筛查感染率为16.90%(5 326/31 523),以高危型HPV感染为主,其中52、16、58型所占比例最高,分别为17.70%,15.04%和12.32%;5 326例HPV阳性标本中,单一感染者4 331例,构成比为81.32%,双重感染为775例,构成比为14.55%,三重或以上感染为220例,构成比为4.13%,最多可7种亚型同时感染;按年龄分组中>60岁组HPV感染率最高,占29.96%,其次为>50~60岁组(22.43%),≤30岁组 (17.61%),>40~50岁组(16.66%),>30~40岁组感染率最低(14.17%),>30岁组感染率有上升趋势。结论 广东省江门市就诊患者HPV感染率较高,最常见的是HPV52亚型,>60岁是HPV感染高发人群,应加强HPV筛查力度,为今后HPV相关疾病的防治提供基础依据。

Abstract:Objective To understand the infection and distribution of human papillomavirus (HPV) in Jiangmen area of Guangdong Province, and provide reference for future prevention and treatment of human papillomavirus infection and cervical cancer. Methods Using gene amplification technology and membrane hybridization principle to detect 31 523 females of Central Hospital in Jiangmen City from January 2015 to December 2017. The results were performed the retrospective analysis for comparing the epidemiological characteristics of different HPV genotypes. SPSS22.0 was used for statistical analysis of the data. Results The infection rate of HPV in Jiangmen area of Guangdong Province was 16.90% (5 326/31 523), the infection rate of high-risk HPV was obviously higher than low-risk HPV, and HPV52, 16, and 58 were the highest, accounting for 17.70% ,15.04% and 12.32% respectively. Among 5 326 HPV-infected specimens, 4 331 were single infections, accounting for 81.32% (4 331/5 326), and 775 were double infections, accounting for 14.55%(775/5 326),220 were triple or more infections, accounting for 4.13%(220/5 326),and the maximum infection was 7 subtypes infected. According to age group, the HPV infection rate was the highest in the group of >60 years old, accounting for 29.96%, followed by >50-60 years old group (22.43%), ≤30 years old group (17.61%), >40-50 years old group (16.66%),the infection rate was the lowest in the >30-40 years old group (14.17%), and the infection rate of >30 years old group had an increasing trend. Conclusion The HPV infection rate of the patients in Jiangmen area of Guangdong Province is high, the most common type is HPV52 subtype. More than 60 years old is a high incidence of HPV infection. The intensity of HPV screening should be strengthened, The present results will provide a fundamental basis for the prevention and treatment of HPV related diseases in the future.
