腹腔镜下经胆囊管胆总管取石30例分析

目的：评价分析30例胆总管结石患者腹腔镜下经胆囊管胆总管取石的临床效果。方法回顾性分析80例2010年3月～2012年3月期间在我院接受治疗的胆总管结石的患者，根据治疗方法的不同分为试验组（30例）和对照组（50例）。其中，试验组患者行腹腔镜下经胆囊管胆总管取石治疗，对照组患者给予传统开腹手术治疗，统计并记录两组患者的平均住院时间、手术时间、术中失血量、手术切口长度及手术有效率，比较分析腹腔镜下经胆囊管胆总管取石的临床疗效。结果试验组的平均住院时间、手术时间、术中出血量、手术切口长度优于对照组，两组的组间差异具有统计学意义（P＜0.05）；试验组的总有效率为100.00%（30/30）优于对照组为98.00%（49/50），两组的组间差异无统计学意义（P＞0.05）。结论腹腔镜下经胆囊管胆总管取石在治疗胆总管结石方面临床优异性显著，且减轻患者术中损伤，改善预后，简化手术步骤，治疗有效率高，值得临床的应用与推广。

Analysis of 30 cases of laparoscopic transcystic common bile duct exploration for treating common bile duct stones

Objective To evaluate the clinical effects of laparoscopic transcystic common bile duct exploration in 30 cases of patients with common bile duct stones. Methods Retrospectively analyzed 80 cases of patients with common bile duct stones treated in our hospital from March 2010 to March 2012,divided them into experimental group（30 cases） and control group（50 cases） according to different treatment methods. The experimental group treated with laparoscopic transcystic common bile duct exploration,the control group adopted conventional abdomen surgery, recorded the average hospital time, operative time,amount of bleeding during operation,incision length and operation efficiency,compared the clinical efficacy of laparoscopic transcystic common bile duct exploration. Results The average hospital time,operative time,amount of bleeding during operation,incision length and operation efficiency of experimental group were better than control group, the differences in two groups were statistically significant（P 〈 0.05）;The total effective rate of experimental group100%（30/30）] was higher than control group98.00%（49/50], the differences in two groups had no statistical significance（P 〉 0.05）. Conclusion Laparoscopic transcystic common bile duct exploration has significant advantages in the treatment of common bile duct stones,which can ameliorate injury of patients during operation and improve prognosis,make the operation steps simple,having high treatment efficiency,it deserves to be promoted and applied in clinic.
