无创正压通气治疗慢性阻塞性肺疾病急性加重合并呼吸衰竭40例临床分析

目的：评价无创正压通气治疗慢性阻塞性肺疾病急性加重期合并呼吸衰竭的临床疗效。方法将2012年10月～2013年8月收治的40例慢性阻塞性肺疾病急性加重期患者按随机数表分为对照组（n=20）和NIPPV组（n=20），两组均给予常规治疗，NIPPV组在常规治疗基础上加用无创正压通气治疗。治疗1周后比较两组患者心率、呼吸频率、血气指标的变化及疗效。结果 NIPPV组的呼吸频率、心率、血气指标的改善水平均显著优于对照组，两组比较差异有统计学意义（P＜0.05）；NIPPV组的治疗效果显著优于对照组，两组比较差异有统计学意义（P＜0.05）。结论无创正压通气结合常规疗法能有效提高慢性阻塞性肺疾病急性加重期的疗效。

Evaluate noninvasive positive pressure ventilation on 40 patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To evaluate the clinical efficacy of noninvasive positive pressure ventilation （NIPPV） for patients with acute exacerbation of chronic obstructive pulmonary disease （AECOPD）. Methods Forty AECOPD patients from 2012 Ocotober to 2013 August were recruited and assigned to standard treatment group（n=20） and NIPPV treatment group（n=15）.NIPPV group was received standard treatment plus NIPPV treatment. The heart rate, respiratory rate and arterial blood gas of two groups were compared. Results The changes of heart rate, respiratory rate and arterial blood gas of NIPPV group were better than standard group.There was a significant difference between the two groups （P 〈 0.05）.The clinical efficacy of NIPPV group was better than standard group. There was a significant difference between the two groups （P 〈 0.05）. Conclusion NIPPV combined standard treatment can improve the clinical efficacy of patients with acute exacerbation of chronic obstructive pulmonary disease.
