胰十二指肠切除术后并发症及相关因素分析

目的探讨胰十二指肠切除术后并发症发生率及相关危险因素。方法回顾性分析188例行胰十二指肠切除术患者临床资料，并对并发症相关因素进行单因素和多因素分析。结果56例发生手术相关严重并发症（29．79％）。单因素分析年龄、术前总胆红素升高、术前低蛋白血症、术前贫血、术前黄疸时间、术前减黄治疗、术中出血多及肿瘤患者肿瘤浸润深度对术后并发症的影响显著（P〈0．05）。多因素分析术前总胆红素、术前白蛋白、术前血红蛋白、术前黄疸时间、术中出血量是术后并发症的独立危险因素（P〈0．01）。结论术前胆红素高、黄疸时间长、低白蛋白血症、贫血及术中失血过多是胰十二指肠切除术后并发症的危险因素．应积极控制危险因素，以改善患者预后。

Analysis of the incidence and risk factors of complications after pancreatieoduodenectomy

Objective To investigate the incidence and risk factors of complications after pancreatieoduodenectomy.Methods The clinical data of 188 cases after pancreatieoduodenectomy were retrospective analyzed,and the related factors of complications were analyzed.Results Fifty-six cases after pancreatieoduodenectomy had occurred complications(29.79%).Univariate analysis,age,preoperative total bilirubin,preoperative hypoproteinemia,preoperative anemia,jaundice before surgery time,preoperative treatment of jaundice,blood loss and the depth of tumor invasion of cancer patients had the significantly impact of complications(P<0.05).Multivariate analysis,preoperative total bilirubin,preoperative albumin,preoperative hemoglobin,preoperative jaundice time,blood loss were the risk factors for complications(P<0.01).Conclusion Preoperative high bilirubin,jaundice for a long time,hypoalbuminemia,anemia and excessive blood loss after pancreatoduodenectomy are risk factors for complications,should be actively controlling risk factors to improve the prognosis of patients.
