SSFSE序列MRCP结合常规MR扫描对肝外胆道梗阻性疾病的诊断价值

目的评价单次激发快速自旋回波（single shot fast spin echo，SSFSE）序列MRCP结合常规MR断层图像对肝外胆道梗阻性病变的诊断价值。方法应用SSFSE序列MRCP对80例肝外胆道梗阻患者进行扫描，同时进行常规MR扫描。观察肝外胆道梗阻性病变梗阻的位置、胆管扩张形态、程度及梗阻端病变的MRCP表现特征；比较MRCP及MRCP结合常规MR对肝外胆道梗阻性病变的定性诊断。结果MRCP定位诊断符合率为100％。定性诊断MRCP准确率为77．5％，MRCP结合常规MR为91．3％。结论SSFSE序列MRCP能充分显示胆道的解剖形态，与常规MR扫描相结合对肝外胆道梗阻性病变的具有较高的诊断价值。

Evaluation of MRCP in the use of single shot fast spin echo sequence in diagnosis of extrahepatic bile duct obstructive disease and the value of combing conventional MRI

Objective: To evaluate the diagnostic value of MRCP in the use of SSFSE sequence,and the value of conventional T2-and T1-weighted sequences in patients with extrahepatic bile duct obstructive disease.Methods: 80 patients with extrahepatic bile duct obstruction disease received SSFSE-MRCP and coventional MR imaging.The characteristic MRCP finding,including duct visualization and morphology of various biliary obstruction diseases,were observed.The efficacy of diagnosis was evaluated for MRCP alone and MRCP combining conventional MR imaging.Results: The accuracy of localizative diagnosis in the use of MRCP was 100%.The accuracy of that diagnosed by MRCP was 77.5%,and the accuracy of that diagnosed by MRCP combining conventional MR imagingwas 91.3%. Conclusion: MRCP in the use of SSFSE can thoroughly demonstrate biliary anatomic form,and combing conventional MRI has high diagnostic value.
