摘    要:目的探讨上腔静脉及三尖瓣血流速度曲线中各波速度及其相关比值在评价容量反应性中的应用价值。方法用超声心动图方法记录16名健康青年男性在模拟低血容量情况[最大-60mmHg(1mmHg=0.133kPa)的阶梯式递增下体负压作用]下上腔静脉及三尖瓣的流速曲线,测量各波速度,计算其相关比值,并进行统计学分析。结果下体负压作用下,上腔静脉流速曲线中的心室收缩波和心房收缩反向波速度与下体负压水平有较好的相关性,相关系数分别为-0.744和-0.771,P〈0.001。结论上腔静脉流速曲线中心室收缩波和心房收缩反向波速度在评价容量反应性中有一定应用价值。

Abstract:Objective To study the value of Doppler blood flow velocity indices of tricuspid valve(TV) and superior vena cava(SVC) in predicting the fluid responsiveness. Methods The SVC and TV Doppler flow patterns of 16 health young men were examined by echocardiography during graded lower-body negative pressure(LBNP) of up to -60 mm Hg ( 1 mm Hg = 0.133 kPa) which simulated the hypovolemia. The peak velocities of all waves and their related ratios, such as S/D, E/A, were measured, calculated and statistically analyzed. Results Peak velocities of wave S and wave AR correlated well with the grades of LBNP (r= -0.744 and r= -0.771 ,respectively,P< 0.001 ). Conclusions The peak velocities of wave S and wave AR are of good value in assessing the fluid responsiveness.
