脑认知功能评价儿童脑积水三脑室底造瘘术后疗效的价值

摘    要:目的探讨用脑认知功能评价儿童脑积水三脑室底造瘘术后疗效的价值。方法2007年6月~2012年6月,对20例行三脑室底造瘘术的脑积水患儿于手术前和术后2、6个月进行智能评测(采用Gesell发育诊断量表,包括适应行为、大运动行为、精细运动行为、语言行为和个人-社会行为)及脑室额角宽度测量,进行术后疗效评价。结果术后2、6个月与术前相比智能测定有统计学差异(P〈0.01),分别有8/20和15/20患儿DQ恢复正常。术后脑室大小较术前差异无显著性(P〉0.05)。结论脑发育的改善及脑认知功能的恢复对儿童脑积水三脑室底造瘘术后疗效的评价有重要意义。

The Significance of the Neurocognitive Function Assessment Evaluation on the Effect of Third Ventriculostomy of Hydrocephalus for Children

Abstract:Objective To discuss the significance of neurocognitive function evaluation of the effect of the third ventriculostomy of children hydrocephalus. Methods Between June 2007 and June 2012, we carried out IQ test on 20 patients before endoscopic third ventrieulostomy and 2 months and 6 months after the surgery ( Gesell Developmental Schedules, including adaptive behaviour, gross movement, fine movement, language behavior and personal-social behavior) and measured their ventricle size to evaluate the effect of the third ventriculostomy of hydrocephalus. Results Significant difference could be found in the patients' intelligence test results between 2 months and 6 months after the surgery and before the surgery ( P 〈 0.01 ). The DQ of 8/20 and 15/20 cases respectively returned to normal. There was no significant difference between the postoperative ventricle size and the preoperative one of the patients (P 〉 0.05). Conclusion The improvement of the brain function and recovery of cognitive function play a significant role in valuating the effect of the third ventriculostomy of children hydrocephalus.
