腹腔镜管状胃胃切除术治疗肥胖症合并2型糖尿病患者的围手术期护理

目的:探讨腹腔镜下管状胃胃切除术(LSG)治疗肥胖症合并2型糖尿病(diabetes mellitus,2型DM)患者的围手术期护理措施,以提高护理质量。方法:选择19例行LSG肥胖症合并2型DM患者,术前加强心理疏导,讲解肥胖及2型DM危害,科普手术知识及风险-效益比;术后加强导管护理及并发症观察,注意饮食和运动指导,强调严格遵食谱饮食及定期随访的重要性。总结LSG治疗肥胖症合并2型DM的围术期护理流程。结果:本组19例患者手术均经腹腔镜完成,无中转开腹,术后均顺利康复出院,围手术期无手术及护理并发症发生。结论:有效的围手术期护理措施是保障手术成功的重要因素,应针对肥胖症合并2型DM患者的疾病及心理特点,采取有针对性的护理措施,促进患者早日康复,提高患者的生存质量。

Perloperative nursing of laparoscopic sleeve gastrectomy for obesity patients with type 2 diabetes mellitus.

Objective： To investigate the effective perioperative nursing measures of laparoseopie sleeve gastrectomy （LSG） for type 2 diabetes mallitus. Methods：19 patients with obesity combined with type 2 diabetes mellitus were included. Before LSG procedure, all the patients underwent the psychotherapy ,got a better understanding of the dangers of obesity and type 2 diabetes mellitus and informed of the procedure and cost - effectiveness. After the proce- dure, catheter care was performed and complications were closed observed. Patients were guided to undertake some exercise and watch their diet. All patients were aware of the importance of sticking to rigid diet and regularly fi）llow - up. On that basis, the procedures of perioperative nursing for obesity combined with type 2 diabetes mellitus were concluded. Results：All cases were successfully operated by laparoscope. There was no conversion. All the patients recovered uneventful and were discharged. No procedure or nursing - related complication was seen during the perioperative period. Conclusion ： The effective pcrioperative nursing was a most important factor for a successful operation. Due to the characteristics of the obesity complicated with type 2 diabetes mellitus and patients＇ mental status, some targeted nursing measures should be adopted to help rehabilitation and improved the patients＇ quality of lives.
